In the previous episode, “A Third Act Surprise,” Watson and his team race to perform a life‑saving kidney transplant on a young woman, Hollis. Since she was conceived with the help of a sperm donor, details about her biological family were lacking. Upon further investigation, Watson learnt that the doctor at the fertility clinic used his own sperm in over 40 patients. The doctor later killed himself, leaving his kidney intact. But since he was an alcoholic, Watson couldn’t use his organ. Meanwhile, Sherlock returns to Pittsburgh, claiming he recovered from a brain tumor.
